In January 2008 the AGWS was successful in obtaining funding through the Department of Human Services to participate in an 18-month pilot project to provide intensive assistance to HACC eligible individuals residing within the North and Western metropolitan regions. Through the project the DHSD is primarily aimed at exploring the definition and functions associated with the role of care coordination in an attempt to formalise funding arrangements with the ethnic sector to continue undertake this role. In particular, the project will;

• Explore and develop mechanisms that will assist HACC eligible people from   

   CALD backgrounds to access and negotiate HACC and other related services;
• Build capacity in order to work effectively within the broader HACC service

   system in the provision of specialist cultural knowledge, information and

   advice;
• Assist generic organisations in developing culturally appropriate service

   responses for specific clients/residents.

Since the project’s inception the organisation has supported up to 60 individuals from Greek-speaking background in the provision of information, advice and supported referral to other relevant organisations. Collaborative working arrangements have been developed and maintained with local government organisations, community health services, the acute and sub-acute sectors, rehabilitation providers, aged care assessment services, VCAT and other providers that are involved in addressing the needs of the HACC target group.
